What is the total annual family income? (Please check one, including the income of both parents, if living together. Please be sure to include all sources of income such as child support, income from other members of the household, etc.)

1. A copy of the first page of your latest income tax return (1040 form) indicating total annual income. 2. Any documentation from federal, state, county, or local assistance program that verifies family need.

Policies

  • Full or partial tuition assistance to attend the Museum Summer Art Camp is based on financial need and is available on a limited basis to eligible students.
  • Tuition assistance will be awarded based on available funding.
  • If your student receives tuition assistance, you agree to provide or make arrangements for transportation to and from the Museum each day of Camp.

Camp Availability

Please select any and all camp weeks that your child is qualified to attend based on age and you are also available to drop them off and pick them up during corresponding camp dates (for more information about camp dates, times and themes, visit our website)
